The CLO Support Professional is an integral part of our Supported Independent Living services. He/She is responsible for working as a team member of the household. The specific duties of the Support Professional will be those defined by the CLO Coordinator. He/She is directly accountable to the CLO Coordinator, and is thereby equally responsible, along with and under the direction of the Director of Individualized Support Options. A CLO program is created to promote independence and personal responsibility for those individuals with basic independent living skills and need support staff to promote success.
ESSENTIAL JOB FUNCTIONS:
- To develop a warm, home-like, cooperative atmosphere in the residence in which both individuals and staff share.
- To safeguard the privacy and confidentiality of the residents and staff.
- To develop and implement the teaching plans.
- Monitor individual compliance with medication compliance and management, as needed.
- Assist individuals to complete routine responsibilities and expectations established within the household
- To perform duties that ensures the health and safety of the individuals and staff.
- To work with the CLO Coordinator/ Manager on the development, updating and implementation of the residents' individual support plans and overall ISP responsibility.
- To maintain appropriate progress notes and pertinent records corresponding to individual ISP and other pertinent record keeping requirements.
- To work with the CLO Coordinator/ Manager on the location and/or development of recreational activities for the individuals.
- To assist the CLO Coordinator/ Manager on assisting individuals with managing their own medical and mental health treatment plans and medical orders; to assist with finding new medical service providers as needed.
- To assist in the training of all individuals in self-preservation skills and the development of fire and emergency procedures, which may require going up and down stairs.
- To follow the regulations as outlined by the agencies responsible for licensing and approving the residence.
- To teach individuals to refine skills in the areas of ADL's, self-help and socialization skills to improve independent living skills
- To assist individuals with participation in recreational events and individuals’ doctor’s appointments when necessary.
- To attend and participate in staff meetings and annual ISP meetings as required
- To attend all required trainings and maintain certifications.
- To represent TILL's residential services to the public.
QUALIFICATIONS:
Must be 18 years of age or older. A minimum of 3 years experience working with high functioning individuals with ASD and DD/ID. A a valid driver's license and car are required. Must possess good written and verbal skills to effectively communicate individual needs and objectives. CPR and First Aid training are a condition of employment.
